What Chemo and Radiation Do to Your Immune System

Cancer treatment saves lives, but it extracts a steep biological price. Chemotherapy and radiation can push the immune system into a state that looks decades older than the patient’s actual age. Immune cells shift away from fresh, flexible “naive” forms toward worn-out, less effective types. Muscles weaken. The gut microbiome changes. Survivors are left fighting not just recovery but accelerated aging — even years after treatment ends.

Researchers call this process immunosenescence, a fancy word for immune-system aging. Cancer survivors show it at unusually high rates. Studies of childhood leukemia survivors have found T-cell profiles that look like those of much older people, with fewer young, adaptable immune cells and more exhausted ones. The same pattern appears in adult survivors across multiple cancer types. It is a hidden cost of surviving that rarely makes the headlines.

What the Roswell Park Pilot Study Found

Dr. Shernan Holtan and his team at Roswell Park Comprehensive Cancer Center in Buffalo, New York, enrolled eight long-term cancer survivors who were either frail or at risk of becoming frail. Eight healthy caregivers served as the comparison group. All participants completed a median of 25 personalized resistance training sessions over 10 weeks. The results, published in the journal Cancers, were notable: by the end of the program, differences in blood and gut biome markers between survivors and healthy controls were no longer detectable.[1]

“This study shows that a structured strength-training program may help cancer survivors regain strength and improve their immune health after treatment,” Dr. Holtan said.[1] That is a carefully worded claim — and it should be. This was a pilot study with a small group. It was designed to generate a hypothesis, not prove one. Still, the signal is hard to ignore: sixteen people, ten weeks, and the biological gap between the sick and the healthy closed enough to become unmeasurable.

The Broader Evidence Behind the Signal

The Roswell Park result does not stand alone. A 2025 review in the Journal of the National Cancer Institute Monographs found support across multiple studies for the idea that regular exercise training can positively change the immune system in cancer patients, including increases in natural killer cell activity.[2] A separate study of breast cancer survivors found that eight weeks of exercise produced measurable shifts in immune cell markers, with researchers noting a possible anti-immunosenescence effect.[3] Earlier work described cancer survivors whose immune cells converted from a worn-out form to a more youthful naive form after a 12-week exercise program.[4]

Roswell Park has been building this case for years. A separate six-month study in multiple myeloma patients found that exercise produced a less-exhausted T-cell profile. The ratio of depleted immune cells dropped significantly after training.[10] A later Roswell Park study of 80 multiple myeloma patients showed resistance training cut fatigue and pain, with the strongest results in those who trained in person.[12] The pattern across these studies is consistent: exercise moves immune markers in the right direction in cancer survivors.

Why You Should Be Excited But Not Certain

Here is where honest science pumps the brakes. Every study mentioned above measures biomarkers — blood counts, cell types, ratios. None of them proves that better immune markers lead to fewer infections, fewer relapses, or longer survival. That is the gap between a promising signal and a proven treatment. The Roswell Park pilot enrolled just eight survivors. Small studies are more likely to produce results that look exciting but do not hold up when tested in larger groups.

Media coverage of studies like this tends to sprint past the caveats. Headlines about “reversing immune aging” are technically grounded in what researchers observed, but they suggest a permanence and completeness that a 10-week pilot study cannot establish. Nobody yet knows whether the immune changes last after training stops. Nobody has run a large randomized trial comparing resistance training to a control group with enough participants to settle the question. Those studies need to happen. Until they do, the honest answer is: the evidence is compelling enough to take seriously, but not strong enough to treat as settled fact. What is settled is that resistance training is safe, improves strength, and carries virtually no downside for cancer survivors. That alone makes it worth doing while researchers figure out the rest.
