Your genes may load the gun for type 2 diabetes, but a landmark 6-year trial just showed that what you eat and how you move can pull it out of your hands entirely.

The 31% Number That Has Researchers Talking
A 6-year analysis of the PREDIMED-Plus trial, published in the Annals of Internal Medicine, found that adults who followed an energy-reduced Mediterranean diet and added regular physical activity cut their risk of developing type 2 diabetes by 31% compared to those who followed a traditional, unrestricted Mediterranean diet. [6] That is not a small number. It translates to roughly 3 fewer diabetes cases for every 100 people in the program. The participants were overweight or obese adults aged 55 to 75 with metabolic syndrome, a group already staring down serious risk.
The lead investigator called diabetes the “first solid clinical outcome” proving that this combined diet and activity approach works as a prevention tool. [2] Harvard researcher Frank Hu went further, saying “modest sustained changes in diet and lifestyle could prevent millions of cases of this disease worldwide.” [3] That is a bold claim, but the data behind it is real. The key word is “sustained.” This was not a 12-week diet challenge. This was 6 years of measured, consistent behavior change in nearly 5,000 people.
What the Body Actually Did Over Six Years
The numbers inside the trial are worth slowing down for. At 12 months, the intervention group showed significant drops in insulin resistance, fasting glucose, and triglycerides compared to the control group. [3] They also lost 126 grams of visceral fat, the dangerous fat packed around the organs, while holding onto more lean muscle mass than the control group as they aged. [5] These are not cosmetic wins. Visceral fat and insulin resistance are two of the most direct biological drivers of type 2 diabetes. The body was measurably changing in ways that matter.
The trial does carry one honest limitation worth naming. Diabetes was a secondary outcome. The trial was originally designed to measure cardiovascular disease events, not diabetes incidence. [1] That does not erase the findings, but it does mean a trial built from the ground up around diabetes as the primary goal would deliver even stronger statistical confidence. That trial has not been done yet. What exists right now is still among the highest-quality evidence available on this question.
Where Genetics Fits Into This Picture
Here is where the story gets more interesting, and more nuanced. The PREDIMED-Plus trial did not sort participants by their genetic risk scores. So the direct question of whether lifestyle changes offset a high genetic predisposition was not tested in this study. That is a real gap. But other research has tackled it head-on. An analysis of the Diabetes Prevention Program found that people with the highest genetic risk scores actually prevented more diabetes cases per year of lifestyle effort than those with lower genetic risk. [12] The absolute benefit was greatest where the genetic burden was heaviest.
A separate study in the Journal of Clinical Endocrinology and Metabolism found that high-genetic-risk participants who made lifestyle changes had a 70% lower risk of developing type 2 diabetes. [10] Lifestyle did not erase the genetic risk entirely. The conversion rate never hit zero. But the gap it closed was enormous. The evidence across multiple trials consistently points in one direction: the people who need lifestyle intervention the most, those with the highest genetic load, also benefit from it the most in raw numbers of cases prevented.
The Honest Bottom Line on Genes Versus Habits
No study has shown that lifestyle changes completely cancel out a high genetic risk for type 2 diabetes. Anyone claiming otherwise is overstating the science. What the evidence does show, clearly and repeatedly, is that the relative reduction in risk from lifestyle change is consistent across all genetic risk levels, and the absolute number of cases prevented is actually largest in the highest-risk group. [12] That is a powerful argument for action, not fatalism. The pharmaceutical industry will keep pushing drugs as the answer. The data keeps pointing back to a simpler, cheaper, and personally empowering truth: eat less processed food, move more, and lose some waist fat. The biology responds.
