Low D, High Surgery Pain: Surgeons Stunned

What The New Breast Cancer Study Found

Researchers tracked 184 women who had a unilateral modified radical mastectomy, a common surgery to remove breast cancer. Before the operation, doctors checked each woman’s vitamin D levels. After surgery, they tracked how much pain each patient reported over the next 24 hours.

Women who walked into the operating room deficient in vitamin D were far more likely to report moderate or severe pain than women with normal levels. After adjusting for other factors, researchers calculated these women had more than three times the odds of significant pain at some point in that first day.

The study appeared in Regional Anesthesia and Pain Medicine in May 2026. It stands out because it followed patients in real time, before and after surgery, rather than just looking back at old records.

Painkiller Use Told The Same Story

Pain scores were not the only measure that moved. Women low on vitamin D also used noticeably more opioid medication to control their discomfort after surgery, compared to women with sufficient levels. That detail matters because opioid use carries its own risks, from dependency to slower recovery.

This is not the first time researchers have connected low vitamin D to heavier opioid use. A separate cohort study of more than 11,000 patients found that people with vitamin D deficiency logged nearly two extra days of opioid use per year and needed noticeably higher doses than people with normal levels. The mastectomy study adds a fresh, specific example to that broader trend.

A Pattern Researchers Have Chased For Years

Scientists have studied the vitamin D and pain connection across many types of surgery, from hip fractures to knee replacements to gallbladder removal. Many of these studies, especially ones that track real patients over time, report the same basic finding: low vitamin D lines up with worse pain and more painkiller use.

The picture is not perfectly consistent, though. One study on rotator cuff surgery patients found no meaningful link between vitamin D deficiency and post-operative pain. Another older cohort study on general surgery patients also failed to find a significant connection between vitamin D and pain scores or opioid needs. That mixed record means the mastectomy findings fit a strong, recurring signal, but not a universally proven rule.

Why This Distinction Matters For Patients

The mastectomy study looked at natural vitamin D levels patients already had, not whether taking a supplement before surgery actually reduces pain. Those are two different questions. Larger trials that test supplementation directly have produced more mixed results, with some showing little to no drop in pain scores.

Still, the practical takeaway is simple enough for anyone facing surgery. Checking vitamin D levels ahead of time is cheap, low-risk, and already common in many pre-surgical workups. Given how consistently deficiency lines up with worse pain outcomes across multiple studies, it looks like reasonable information for patients and doctors to have before an operation, not a guaranteed fix, but a smart piece of the puzzle.
